REGISTRATION INCLUDES:  2 Continental breakfasts, all breaks, and lunch on Tuesday, February 25.

REGISTRATION COSTS: (per attendee, this form can be used for multiple attendees).

• Payment by Credit Card - $104.00 (includes transaction fee) if registered prior to February 11 or $125.00 if after February 11.

• Payment by Check - $100.00 if registered prior to February 11 or $125.00 if after February 11.
